This report is based on a report prepared by the Prevention Institute for the Robert Wood Johnson Foundation in June 2015 titled "Measuring What Works to Achieve Health Equity: Metrics for the Determinants of Health."

The initial release of this report on FLHealthCHARTS occurred August 30, 2018. Forthcoming versions will show additional measures and expanded features.

1When the Racial Residential Segregation Index is less than 0.3 the county’s population is “well integrated.” Values between 0.3 and 0.6 indicate the county’s population is “moderately segregated.” Values above 0.6 indicate the county’s population is “very segregated."
